Self-monitoring Program

Social and healthcare service providers in welfare areas and those offering services in more than one service unit must create a self-monitoring program for their operations. The program defines the methods used to ensure that services are arranged and implemented in accordance with agreements and legislation. The self-monitoring program outlines how the delivery of services, safety, quality, and equality are monitored, and how deficiencies are addressed. The self-monitoring program includes self-monitoring plans and medication treatment plans, as specified by law.

Self-Monitoring Plan

A self-monitoring plan must be created as a daily working tool for the service unit, in which the service provider records the measures for ensuring and developing quality.
In the self-monitoring plan, the service provider describes the key risks associated with delivering the services within the service unit and how the provider and staff monitor and ensure the quality, appropriateness, safety of the services, and the sufficiency of staff involved in customer and patient work.
